HIV-related practices and ethics - survey of opinions in a paediatric department
Seventy-five doctors· working in an academic paediatric department each completed an anonymous se"-administered questionnaire. Questions were posed relating to the need for consent before human immunodeficiency virus (HIV) testing and·the informing of sexual partners of HIV-infected mothers. Only 9% of the doctors thought that the sexual partner of an HIV-infected mother should never be informed if the mother refused to do so. Sixty-one per cent of the doctors thought that pre-test consent was never necessary when screening hospital admissions. This opinion conflicts with the view of the South African Medical and Dental Council that pre-test consent is mandatory
Collective polarization exchanges in collisions of photon clouds
The one-loop "vacuum" Heisenberg-Euler coupling of four electromagnetic
fields can lead to interesting collective effects in the collision of two
photon clouds, on a time scale orders of magnitude faster than one estimates
from the cross-section and density. We estimate the characteristic time for
macroscopic transformation of positive to negative helicity in clouds that are
initially totally polarized and for depolarization of a polarized beam
traversing an unpolarized cloud.Comment: Recapitulates much that is in hep-ph/0402127, with new results in the

Impact of aging on the auditory system and related cognitive functions: A narrative review
Age-related hearing loss (ARHL), presbycusis, is a chronic health condition that affects approximately one-third of the world’s population. The peripheral and central hearing alterations associated with age-related hearing loss have a profound impact on perception of verbal and non-verbal auditory stimuli. The high prevalence of hearing loss in the older adults corresponds to the increased frequency of dementia in this population. Therefore, researchers have focused their attention on age-related central effects that occur independent of the peripheral hearing loss as well as central effects of peripheral hearing loss and its association with cognitive decline and dementia. Here we review the current evidence for the age-related changes of the peripheral and central auditory system and the relationship between hearing loss and pathological cognitive decline and dementia. Furthermore, there is a paucity of evidence on the relationship between ARHL and established biomarkers of Alzheimer’s disease, as the most common cause of dementia. Such studies are critical to be able to consider any causal relationship between dementia and ARHL. While this narrative review will examine the pathophysiological alterations in both the peripheral and central auditory system and its clinical implications, the question remains unanswered whether hearing loss causes cognitive impairment or vice versa
Robust autoresonant excitation in the plasma beat-wave accelerator: a theoretical study
A modified version of the Plasma Beat-Wave Accelerator scheme is introduced
and analyzed, which is based on autoresonant phase-locking of the nonlinear
Langmuir wave to the slowly chirped beat frequency of the driving lasers via
adiabatic passage through resonance. This new scheme is designed to overcome
some of the well-known limitations of previous approaches, namely relativistic
detuning and nonlinear modulation or other non-uniformity or non-stationarity
in the driven Langmuir wave amplitude, and sensitivity to frequency mismatch
due to measurement uncertainties and density fluctuations and inhomogeneities
Closed formula for the relative entropy of entanglement in all dimensions
The relative entropy of entanglement is defined in terms of the relative
entropy between an entangled state and its closest separable state (CSS). Given
a multipartite-state on the boundary of the set of separable states, we find a
closed formula for all the entangled state for which this state is a CSS. Quite
amazing, our formula holds for multipartite states in all dimensions. In
addition we show that if an entangled state is full rank, then its CSS is
unique. For the bipartite case of two qubits our formula reduce to the one

The horofunction boundary of the Hilbert geometry
We investigate the horofunction boundary of the Hilbert geometry defined on
an arbitrary finite-dimensional bounded convex domain D. We determine its set
of Busemann points, which are those points that are the limits of
`almost-geodesics'. In addition, we show that any sequence of points converging
to a point in the horofunction boundary also converges in the usual sense to a
point in the Euclidean boundary of D. We prove that all horofunctions are
Busemann points if and only if the set of extreme sets of the polar of D is

Membrane Type 1 Matrix Metalloproteinase Regulates Monocyte Migration and Collagen Destruction in Tuberculosis
Tuberculosis (TB) remains a global pandemic and drug resistance is rising. Multicellular granuloma formation is the pathological hallmark of Mycobacterium tuberculosis infection. The membrane type 1 matrix metalloproteinase (MT1-MMP or MMP-14) is a collagenase that is key in leukocyte migration and collagen destruction. In patients with TB, induced sputum MT1-MMP mRNA levels were increased 5.1-fold compared with matched controls and correlated positively with extent of lung infiltration on chest radiographs (r = 0.483; p < 0.05). M. tuberculosis infection of primary human monocytes increased MT1-MMP surface expression 31.7-fold and gene expression 24.5-fold. M. tuberculosis-infected monocytes degraded collagen matrix in an MT1-MMP-dependent manner, and MT1-MMP neutralization decreased collagen degradation by 73%. In human TB granulomas, MT1-MMP immunoreactivity was observed in macrophages throughout the granuloma. Monocyte-monocyte networks caused a 17.5-fold increase in MT1-MMP surface expression dependent on p38 MAPK and G protein-coupled receptor-dependent signaling. Monocytes migrating toward agarose beads impregnated with conditioned media from M. tuberculosis-infected monocytes expressed MT1-MMP. Neutralization of MT1-MMP activity decreased this M. tuberculosis network-dependent monocyte migration by 44%. Taken together, we demonstrate that MT1-MMP is central to two key elements of TB pathogenesis, causing collagen degradation and regulating monocyte migration
